Study Background

Nemours Children’s Health researchers are studying young adults with type 1 diabetes to learn how to best support their transition from pediatric to adult healthcare.

Type 1 diabetes can be challenging for young adults, especially when transitioning from pediatric to adult care. This study aims to better understand how young adults manage this transition and how additional support can improve health outcomes, engagement with care, and confidence in managing their diabetes. We will examine how specialized Transdisciplinary Care for Transition (TCT) compares to standard care, including its impact on blood sugar control, diabetes-related stress, and successful continuation of care during this critical period.

The knowledge gained from this study may help improve strategies for supporting young adults with type 1 diabetes, ultimately promoting healthier outcomes and a higher quality of life.

Additional Information

In this research study, we are trying to understand how young adults with type 1 diabetes transition from pediatric to adult care. We aim to better understand how support from Transdisciplinary Care for Transition (TCT) affects diabetes management, stress, and confidence in navigating adult healthcare. 

You may be eligible for this study if you meet the following criteria:

Inclusion Criteria:

  • Ages 17–25
  • Diagnosed with type 1 diabetes (T1D) for at least 6 months
  • Near the final visit with your pediatric T1D healthcare provider before transitioning to adult care
  • Fluent in English

 

Exclusion Criteria:

  • Another systemic chronic medical illness, except celiac disease, autoimmune thyroiditis, microalbuminuria, hypertension, or well-managed asthma
  • Developmental disability that limits independent living

The study lasts 12 months. Once enrolled, participation involves:

  • Virtual visits: You will complete three evaluations over the 12-month study period—at enrollment, 6 months, and 12 months. Each evaluation includes online surveys about your health, diabetes management, transition readiness, and diabetes-related stress.
  • From home: You will provide blood samples using mailed HbA1c kits. With these kits, you’ll provide a small finger-prick blood sample and mail it back in a prepaid envelope. If needed, we may also request a copy of recent HbA1c results from your pediatric T1D provider.
  • Virtual support sessions (if randomized to TCT): Participants in the Transdisciplinary Care for Transition (TCT) group will attend three virtual support sessions via Microsoft Teams to help prepare for and navigate the transition to adult care. These sessions focus on managing challenges, expectations, and communication in adult T1D care.

As a participant, you may receive up to $200+, as well as transportation costs.
