Transdisciplinary Care for Individuals with Type 1 Diabetes Transitioning to Adult Healthcare

The purpose of this study is to evaluate whether Transdisciplinary Care for Transition (TCT) can effectively support individuals moving from pediatric to adult healthcare.

TCT is a new intervention where individuals receive additional guidance from a team of specialized providers to ease the transition between healthcare systems.

Participants will complete an online survey and provide a finger stick blood sample that you can do at home three times over 12 months. Some participants will also attend three TCT visits within six months of their final pediatric healthcare appointment.

Additional Information

The purpose of this study is to examine how meeting with a diabetes educator, psychologist, and social worker during the transition from pediatric to adult diabetes care affects establishing care with an adult provider and improving health outcomes, such as average blood sugar levels over time (measured by HbA1c).

You may qualify for this study if you meet the following criteria.

Eligibility Criteria:

  • Diagnosed with type 1 diabetes (T1D) for at least 6 months
  • Near the final visit with pediatric type 1 diabetes healthcare provider before transitioning to adult care
  • Ages 17+
  • Speak English
  • No other systemic chronic medical illness except celiac disease, autoimmune thyroiditis, microalbuminuria, hypertension, or well-managed asthma
  • No developmental disability limiting independent living

If you agree to participate, we will ask you to complete three evaluations over the 12-month study period: once at enrollment, once 6 months after enrollment, and once 12 months after enrollment. The evaluations are done virtually and include 1) online surveys and 2) HbA1c measurement. We will also ask participants to complete an interview at about 6 months after enrollment.

You have a 50-50 chance of being randomly assigned to one of two groups.

  1. Half of the participants will be randomly assigned to follow the standard of care for transition as they move from pediatric to adult diabetes care.
  2. The other half will be assigned to participate in three virtual sessions with a diabetes educator, psychologist, and social worker after their last visit to diabetes care at Nemours. The first two visits will take place before you transition to adult healthcare, 1 and 2 months after you sign up for the study. The first TCT visit will focus on things that might be getting in the way of successfully transitioning to adult care. The second TCT visit will focus on expectations and communication in adult care. The third TCT visit will take place within one month of your first visit in adult T1D care and will focus on concerns or problems with adult T1D care.

Participants will be paid up to $150 for completing surveys and dried blood spot A1C kits for evaluations 1, 2, and 3. They will receive an additional $10-$30 for completing other study tasks including signing releases of information and participating in a qualitative interview. You will receive a Greenphire ClinCard, which is a debit card, to which we will upload payments.
