This study is being conducted to better understand how heart failure and type 2 diabetes affect the body and to explore how the study medication may improve heart function, metabolism, and overall health.

You may be eligible for this study if you meet the following criteria:

• Diagnosed with congestive heart failure
• Diagnosed with type 2 diabetes
• Between the ages of 18 and 80
• Currently taking medication (pills) for your heart condition

Overall Duration

The study lasts about 14 weeks. Participants will attend up to 16 study visits during this time.

In-Lab Activities

During the study, participants will visit the research center for screening, health assessments, and follow-up appointments. Study visits may include:

• A physical exam and blood tests
• Heart MRI scans
• A DEXA body composition scan
• An oral glucose tolerance test (OGTT)
• Health measurements and study questionnaires
• Receiving either the study medication (empagliflozin) or a placebo for 12 weeks, followed by an 8-day treatment period with acipimox

Participants will be randomly assigned to one of two study groups. Researchers will collect information from these visits to learn how the study medication affects heart function, metabolism, and blood sugar in people with heart failure and type 2 diabetes.

At-Home Activities

Participants will take the assigned study medication as directed between study visits. They may also be asked to follow study instructions provided by the research team and attend all scheduled appointments.

Participants may receive up to $2,000 for completing the study.
